A proposal by primary care physicians to organize care based on the Institute of Medicine's definition of patient care: "providing care that is respectful or and responsive to individual patient preferences, needs, and values, and ensuring that patient values guide all clinical decisions". The Patient-Centered Medical Home (PCMH) is an approach to providing comprehensive primary care for children, youth and adults. The PCMH is a health care setting that facilitates partnerships between patients, and their personal physicians, and when appropriate, the patient's family. The concept of PCMH may be confusing and complicated, but thanks to the built in functionality of My WorkspaceSM it is a realistic and manageable goal to set for your practice.
